1. Replace the file
Defs.tex
with the text below, and
2. Make the following changes in 
LetterForm.tex
  A. Comment out the line
\def\ReviewForms{\ReviewForm \Comments \Ack}
by iserting a  %  before it, and add the following line next to it
\def\ReviewForms{}
  B. Comment out the line
\def\footone{THE INSTITUTE OF ELECTRICAL AND ELECTRONICS ENGINEERS, INC.}
by iserting a  %  before it, and add the following line next to it
\def\footone{{}}


To adapt the system to other SIAM journals, in addition to all the above
3. Make the following change in the NEW
Defs.tex
Change the line
\def\tac{SIAM Journal on Control and Optimization}
so that it reflects the Journal you have in mind.
